POSITION SUMMARY The Diabetes Program Director is a member of the interdisciplinary healthcare team responsible for improving the care and outcomes of our extensive patient population. The director provides direct supervision over the Certified Diabetes Educator/s and leads the Diabetes Committee to develop and implement a high quality program for the specific patient populations, for both clinical outcomes and customer service in an efficient manner. The Diabetes Program Director would be knowledgeable of the latest clinical trends, best practices, regulatory requirements, and reimbursement patterns for patients in the specialty program. This role will lead the efforts to achieve and maintain The Joint Commission Certification in Diabetes Management at MLKCH.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Utilizes specialized knowledge to plan, develop, and implement policies/procedures, program design, nd educational program of the Diabetes Program.
Works in collaboration with the MLKCH Foundation for philanthropic planning of the Diabetes Program.
Develops evidence based outcomes and goals to measure the success of the Diabetes Program.
Utilize American Diabetes Association assessment tool to predict measurable risk stratification for those at risk of diabetes.
Ensures that safe delivery of quality clinical care according to defined clinical standards/case management plans.
Establishes and maintains a communication system with patients, staff, physicians, and care management related to care provision in an effort to proactively address patient needs.
Acts as a spokesperson for the program and develop awareness in the community and market area through civic groups, business organizations, and other targeted groups.
Plans and implements events in collaboration with Population Health staff to educate the public and staff about prevention and treatment of diabetes, e.g., health fairs, community education programs, and seminars in coordination with MLKCH Marketing & Communications Department and Community Benefit office.
Monitors and/or assesses awareness of local, state, and national standards that affect programs/service lines and/or departments.
Designs, formulates, and implements action plans based upon quality outcomes.
Identifies ideas and insights into clinical improvements.
Facilitates the development of the care continuum ensuring the incorporation of quality outcome indicators and cost efficient parameters.
Crafts collaborative relationships with other departments, organizations, and community organizations to facilitate and support the mission, vision, and business plan.
Identifies and directly confronts issues which affect quality of service; collaborates with managers/physicians to resolve these issues.
Monitors, maintains, and analyzes program quality indicators for performance trends and opportunities; formulates, designs and recommends programs to improve outcomes of measured by program quality indicators in consultation with Quality Improvement department.
Consults/Liaisons with other departments, service lines and niche programs as appropriate.
Incorporates knowledge of external factors and healthcare policies which affect reimbursement for services.
Utilizes collaborative communication techniques to achieve positive outcomes in all interactions.
Uses communication strategies with individuals/groups that result in the achievement of intended outcomes.
Facilitates the integration of appropriate personnel into teams and committees.
May participate in contracting activity with Managed Care Department.
Provides consultation related to diabetes program development.
Effectively manages the change process.
Prepares accurate and timely written reports, statistics and other documentation, as necessary or as requested.

POSITION REQUIREMENTS
A. Education:
Master’s Degree in Nursing or Clinical Nutrition or Pharmacy required. Doctoral Degree preferred.
B. Qualifications:
Minimum 2 years as a Certified Diabetes Educator/Practitioner and/or 3 years progressive leadership skills. Minimum 2 years disease specific designation program development/management experience. CA Registered Nurse License or CA registered dietician or CA licensed pharmacist in good standing required. BLS Diabetes Education Certification required.
C. Special Skills/Knowledge
Past experience preferred overseeing, developing, or implementing a disease specific program or comparable experience.
